1. Who we are:
Lebone Medical Supplies (Pty) Ltd t/a Amtronix Diagnostics was established in 2007 as a company in the private tertiary sector. The name of the company has recently changed to t/a Amtronix Diagnostics. We focus on providing hearing as well as balance related medical devices and services to hearing health care professionals. Amtronix Diagnostics is the sole supplier of Maico, Natus, Otometrics and Path Medical equipment in Africa.
2. Type of personal information we collect:
We currently collect and process the following information:
- Personal identifiers, contacts, and characteristics (personnel name, position, contact details).
- Company details (physical address, account details, contact details, location information).
3. How we collect the personal information and what it is used for:
Most of the personal information we process is provided to us directly by you for one of the following reasons:
- Quoting, accounts, service agreements, and sale orders.
- Provision of technical support (telephonic & remote)
We do not receive personal information indirectly.
We use the information that you have provided to us in order to:
- Generate quotes, works orders, promotional/marketing documents, and invoices.
- Perform technical services on equipment.
- Generate service agreements/contracts, product fault reports, promotional
In special circumstances, we may share this information with:
- The South African Government (in the event of a legal obligation)
4. When we will process your personal information:
In terms of the Protection of Personal Information Act (POPIA), the justification ground which we base our processing on consist out of the following:
- The data subject or a competent person where the data subject is a child consents to the processing. Kindly note that you have the right to withdraw your consent;
- The processing is necessary to carry out actions for the conclusion or performance of a contract to which the data subject is a party;
- The processing complies with an obligation imposed by law on the business;
- The processing protects a legitimate interest of the data subject;
- The processing is necessary for the proper performance of a public law duty by the business; or
- The processing is necessary for pursuing the legitimate interests of the business or of a third party to whom the information is supplied.
5. How we store your personal information:
Your information is securely stored:
- Electronic records and scanned physical records are stored on the Company server, which is thoroughly protected with up-to-date safeguards to prevent any form of breach or attack from malicious entities.
- Physical records are stored on the Company premises in secure locations (protected with necessary physical safeguards such a biometric magnetic lock system, access control, etc) to prevent any form of breach or data leak.
Amtronix has put into place a data destruction policy concerning the destruction of both physical and electronic data to ensure all data necessary for destruction/deletion is done so in a fitting manner.
We keep your information for: accounting purposes (such as: customer contact details, location information, VAT records, tax records, invoices, credit agreements, sale orders, etc), service provision (such as: contact information, locationinformation, device information, etc).
6. Your data protection rights:
You are entitled to access your personal data by sending a written request to Shawn Hardy (shawn@amtronix.co.za) we may charge you a fee for this service as set out in terms of Condition 8 of POPIA.
You may also request Lebone Medical Supplies (Pty) Ltd t/a Amtronix Diagnostics to correct or supplement any of your personal data which Lebone Medical Supplies (Pty) Ltd t/a Amtronix Diagnostics will undertake as soon as practicable.
Request the destruction of your personal data. Shawn Hardy will consider your request in light of any other laws or regulations prohibiting Lebone Medical Supplies (Pty) Ltd t/a Amtronix Diagnostics from destroying your personal data.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you. Please contact us at shawn@amtronix.co.za if you wish to make a request.
